General Release Waiver *

I acknowledge that I am registering my child to participate in sports activities organized by Zaxon Group LLC. I understand these activities involve inherent risks, including but not limited to bodily injury, property damage, and death. Accordingly, I agree to the following:

Release and Waiver of Liability: I hereby release and forever discharge Zaxon Group LLC, its directors, officers, agents, employees, and volunteers (collectively, the "Released Parties") from any and all liability, claims, demands, and causes of action that I or my child may have now or in the future, arising out of or related to any loss, damage, or injury, including death, that may be sustained by my child, whether caused by the negligence of the Released Parties or otherwise.

Assumption of Risk: I understand that my child’s participation in activities organized by Zaxon Group LLC carries certain risks. I voluntarily assume full responsibility for those risks on my child’s behalf.

Indemnification: I agree to indemnify, defend, and hold harmless the Released Parties from any and all claims, damages, or legal actions brought on behalf of my child in connection with participation in these activities.

Medical Treatment: I authorize Zaxon Group LLC, its agents, and volunteers to obtain or provide emergency medical care for my child in the event of illness or injury. I accept responsibility for all costs related to such treatment.

Acknowledgment of Understanding: I have read this waiver, fully understand its content, and acknowledge that by agreeing to it, I am waiving certain rights on behalf of my child. I confirm I have the legal authority to do so.

Photo & Media Release

Media Use Authorization: I, as the parent or legal guardian of the participant, grant permission to Zaxon Group LLC to capture and use my child’s image or likeness in photos, videos, or other media for promotional and educational purposes.

Where Images May Appear: These materials may appear in league websites, social media, printed materials, or other digital publications. I understand that no compensation will be provided for the use of these images.

Ongoing Permission: This authorization remains in effect unless I notify Zaxon Group LLC in writing that I wish to revoke it.
